Email Chyna Laurence – Chyna.Laurence@sportsinaction.com – your district W-9 form with any additional information (additional forms needed to be filled out, specific departments receiving the funds, etc.) and she will process your check request!

NOTE: All donation forms received BEFORE April 1st, 2025 will be distributed on May 1st, 2025 to arrive in time before the end of the school year.

All donation forms received after April 1st will be distributed during the 2025-26 school year.
